Building the Community-Provider Partnership for Gender-Affirming Primary Care

The purpose of ESSOGIE program is to advance social, political and economic equality with respect to sex, sexual orientation, gender identity or expression. Advancement towards a greater understanding of the intersection of sex and gender with other identity factors that include race, national and ethnic origin, Indigenous origin or identity, age, sexual orientation, socio-economic condition, place of residence and disability are encouraged under the Program. This 16-month project will strengthen the capacity of Hamilton Trans Health Coalition Inc. to advance LGBTQ2 equality. This will be achieved by strengthening collaboration among coalition members and care providers (especially family physicians); increasing access to evidence, data, information, and knowledge sources; developing a mentorship program for professionals in the health care system; amplifying sector capacity at large; advancing LGBTQ2 equality with an intersectional approach; providing health care professionals with the knowledge base needed to address trans, gender-diverse, and non-binary people’s health needs; and ensuring long-term sustainability through formalized support, mentorship and requisite learning. At the end of the project, the organization will have increased knowledge and capacity to advance equality with respect to sex, sexual orientation, gender identity and expression; and established and strengthened partnerships and collaborations to support the LGBTQ2 movement.
